What is the GEMCO Medicare Part B DME Drop‑Ship Program?

The GEMCO Medical Medicare Part B DME Drop‑Ship Program enables pharmacies to provide continuous glucose monitors (CGMs) and other diabetes products to Medicare Part B patients without the need to carry inventory. The pharmacy remains the billing entity, submitting Medicare Part B claims for reimbursement after the patient receives their shipment from GEMCO Medical.

 

There are no minimum purchase requirements, and each shipment is fully customized with your pharmacy’s logo and information, creating a seamless patient experience—as if the product were shipped directly from your pharmacy.
NOTE: The pharmacy must have a Med B license.

Does GEMCO bill Medicare or the patient?
No. GEMCO Medical does not bill Medicare or the patient. The pharmacy remains the billing entity and submits the Medicare Part B claim for reimbursement after the patient receives their shipment from GEMCO Medical. Each shipment is fully customized with your pharmacy’s logo and information, creating a seamless patient experience—as if the product were shipped directly from your pharmacy.
Who owns the patient relationship?
The pharmacy does. This is not a referral or patient extraction program. Your pharmacy remains the provider of record and continues to manage the patient relationship and ongoing care coordination.
Do I need to carry inventory?

No, you do not have to carry inventory. GEMCO Medical ships diabetes supplies directly to patients through customized, just‑in‑time deliveries—branded with your pharmacy’s logo and information on both the shipping label and packing slip. This approach eliminates inventory carrying costs, reduces storage concerns, and minimizes product expiration risk.

 

How quickly are CGMs and other products delivered to patients?
Once an order is validated, GEMCO typically delivers the shipment directly to the patient within one to three business days.
Who is responsible for documentation and eligibility review?
The pharmacy is responsible for gathering all required documentation, obtaining chart notes or Certificates of Medical Necessity (CMNs) from the prescriber as needed, and verifying Medicare Part B eligibility prior to submitting the order to GEMCO Medical.
How does order tracking and proof of delivery work?

GEMCO Medical provides order confirmation, tracking information, and proof of delivery (POD) through its online portal. This documentation supports pharmacy billing and compliance requirements.

How does billing work with Medicare Part B?
After delivery, the pharmacy submits the Medicare Part B claim to CMS using its existing billing process. GEMCO Medical invoices the pharmacy separately for the product and applicable shipping and handling fees.
Are there limits on how often CGMs are shipped?
The program supports up to four shipments per year for ongoing CGM therapy. Eligible orders may be shipped up to 10 days early, with billing aligned to Medicare days‑supply requirements.
What are the requirements to participate?
To participate in the program, pharmacies must establish an account with GEMCO Medical and complete the onboarding process. Participating pharmacies must be properly enrolled in Medicare Part B and hold all required state DME/HME licenses in order to bill and ship products. In addition, pharmacies must execute a Business Associate Agreement (BAA) with GEMCO Medical in compliance with HIPAA regulations.

How do I get started?
In order to purchase from GEMCO Medical, a business account is required. We do not sell directly to consumers. To set up your new account accurately and promptly, please complete our New Account Application, or, if you prefer, download a PDF version.

 

Once the application is submitted, a GEMCO Medical account representative will contact you to discuss the program, review the requirements, and begin expediting your onboarding.
